An accident happened in Dent County on Highway 119 about two and a half miles from Route E Monday afternoon at 3:45. According to the highway patrol report, a 2003 Chevy Trailblazer driven northbound by 33-year old Crystal Crunk of Bland, traveled off the right side of the roadway. Crunk overcorrected twice before going back off the right side of the road where the Trailblazer struck a tree. Crunk suffered moderate injuries and was transported to the Salem Memorial District Hospital by the Salem Ambulance. Crunk was wearing a seat restraint at the time of the accident. 

An accident happened in Reynolds County Monday morning at about 9:00 on Reynolds County Road 752 about six miles south of Reynolds. According to the highway patrol report, a 2002 Hyundai Sante Fe driven northbound by 78-year old Harold Hackworth of Boss, traveled off the right side of the roadway, went down an embankment, struck a fence and then the vehicle overturned. Hackworth suffered minor injuries and was taken by a private vehicle to the Salem Memorial District Hospital. He was not wearing a seat restraint at the time of the accident.

The Salem Memorial District Hospital Board of Directors held their monthly meeting Tuesday evening in the hospital library. Following the audit report from Eddie Marmouget from BKD, the board heard the summary of operations for the month of September. The report showed inpatient admissions were down while outpatient registrations were up as well as emergency room visits. Even though expenses were held in check, the loss of revenue from fewer in-patient admissions caused a loss from operations of $8,454. The cash flow statement showed a decrease in cash of $151,367. The days in accounts receivable in September was 54, which is down from 55 in August. There were 45 acute care patients admitted with an average of 5.3 patients per day. In the Director of Nursing report for the month of September, it was reported there were 905 emergency room visits with 56 transfers. The average turn around time in the ER was 118 minutes for the month. There were a total of 222 ambulance runs with 240 patients being assessed and 168 patients being transported, 30 surgery cases, 1,779 outpatient registrations, 264 Home Health visits, 9,540 lab tests, 1,022 radiological procedures, 183 CT scans, 32 MRI scans, only 6 nuclear med scans due to an inoperable scanner for a few days, 1,325 cardiopulmonary procedures, 450 rehab treatments, 172 dialysis treatments and 330 clinical visits. Hospital Administrator Dennis Pryor reported the hospital auxiliary has one remaining fundraiser, their linen sale, which will be on November 3rd and 4th at the ONCRC. The hospital reviewed the patient satisfaction survey results for the months of July through September for the emergency room, in-patient services, and the rural health clinics. Results were outstanding in all areas. Pryor reported that an internal medical physician would be visiting the hospital next week, while a second physician will be visiting the week of November 10th. Assistant Administrator Kasey Lucas reported the hospital received a fix from CPSI for the continuity of care document for the patient portal. The hospital is running tests on the fix. Because of the problem with the patient portal, the hospital did not meet the MU phase 2 Attestation. After a discussion with the auditors, the hospital voted later to implement the Stage 2 Flexibility Rule to allow them time to correct the problem and meet the Stage 2 Attestation at a later date. In the IT risk assessment about 100 pages of recommendations were created and they will start to address the high-risk items first. Lucas reported that Christine Cottrell, the new Clinical IT Coordinator started on Monday, October 20th. Cindy Fry, RN, went over the Ebola preparation time-line of the hospital, the protocol that is followed at the hospital, and gave a demonstration of the personal protective covering that is required of all employees who may come into contact with patients. Lucas recommended BKD Consulting Services for hire to help with reconciling documentation and assist in the process of securing $440,000 from Medicaid Attestation, which would be a retro attestation, and a 2014 Meaningful Use Flexibility Rule Attestation. The cost would be $45,000 and the board approved this. He also recommended using True Bridge for their electronic pay stubs and year-end payroll W-2’s. The board approved this recommendation. The board approved the following purchases: an Invacare Reliant450 Patient Lift for Dialysis for $2,037 from Direct Supply, a Ricoh copier for the Family Health Center for $5,557, and an FM Scanner for Information Systems from Tiger Direct for $872. The board then went into closed session.

The Salem Lady Tiger Volleyball Season came to an end Monday night as they lost in two straight games to Sullivan in District play in Union. The Lady Volleyball Tigers finished the season with an 0-27-1 record.

Four Salem Lady Softball players were named to the Class 9 District team. The four girls are Shelby Carr, Payton Curley, Camryn Pryor and Savannah Hanning. Congratulations ladies!

Madison Bumgarner added to his already extensive postseason resume with another masterful performance in Game 1 of the World Series on Tuesday. The ace left-hander threw seven innings of one-run ball and got plenty of run support as the San Francisco Giants cooled off the streaking Kansas City Royals, 7-1, to take an early series lead in the Fall Classic. Salavdor Perez hit a solo homerun while James Shields was knocked around for seven hits and five runs over only three-plus innings. The Royals lost for the first time since Sept. 27 and will look to bounce back tonight when rookie Yordano Ventura toes the rubber opposite Jake Peavy. Kansas City had won 11 straight postseason games dating back to the club's only World Series title in 1985, but what would have been a record-tying 12th quickly slipped through the AL champions' fingers. First pitch is set for 7:07 tonight at Kauffman Stadium.

Funeral services for Nora Opal Woolsey of Springfield, formerly of Salem, age 89, will be held Thursday at 10:00 at the James and Gahr Mortuary in Salem. Visitation will be Wednesday from 5:00 to 7:00. Interment will be in the Cedar Grove Cemtery.

Graveside services for Mr. Raymond Z. Wells of Licking, age 74, will be held Thursday at 11:00 at the Boone Creek Cemetery. Arrangements are under the direction of the Fox Funeral Home in Licking.

Funeral services for Pearl Wells of Salem, age 100, will be held Thursday at 2:00 at the James and Gahr Mortuary in Salem. Visitation will be from noon until service time. Interment will be in the North Lawn Cemetery. Memorials may be made to the First Baptist Church of Salem.

Funeral services for Virginia Leora Hewkin, age 80, will be held at 11:00 Friday morning at the Britton-Bennett Funeral Home in Steelville. Visitation will be Thursday from 4:00 to 8:00. Interment will be in the Martin-Trask Cemetery.
